hp computer rebate question?

it's a mail-in rebate, and the form asks me for a PC Model & serial #.
but the thing is, they write it out like this:

PC MODEL & SERIAL

theres only one line....
do i write something

a) Yes ; just write Model: S/N:

b) Just write N/A (not applicable)
